CM Almy provides the following benefits to all full-time employees:
HEALTH INSURANCE
CM Almy Employees are eligible to participate in CM Almy's health plan. There are two medical plans available: Plan B offers Basic coverage; Plan A offers richer coverage at a higher premium, both plans include prescription and preventive care coverage. Almy pays two-thirds of the cost of this benefit.
DENTAL INSURANCE
CM Almy's dental insurance provides 100% coverage for preventive services. Other services are subject to a $50.00 deductible, and are paid at either 50% or 80%. Participants can visit any dentist they choose. Almy pays two-thirds of the cost of this benefit.
FLEXIBLE SPENDING ACCOUNTS
CM Almy offers its employees the opportunity to participate in medical and dependent care flexible spending accounts. These accounts allow employees to use pre-tax dollars to pay for unreimbursed medical and dental expenses, and dependent care expenses.
Full-time Almy employees are eligible for health and dental coverage, and to participate in the flexible spending plan on the first day of the month after sixty days of continuous employment.
LIFE INSURANCE Almy employees are covered by term life insurance based on their salary level. Almy pays for 100% of this benefit.
SHORT TERM DISABILITY
CM Almy provides short-term disability coverage for all employees. This coverage provides up to 50% income replacement during certified disability for up to 26 weeks. Almy pays for 100% of this benefit.
LONG TERM DISABILTY Almy's full-time exempt employees are eligible for long term disability coverage. This coverage provides up to 60% income replacement for a certified disability up to the age of 65. Almy pays for 100% of this benefit.
| | SUPPLEMENTAL LIFE AND DISABILITY Almy employees have the opportunity to purchase additional life and disability insurance at discounted rates.
FULL-TIME ALMY EMPLOYEES ARE ELIGIBLE FOR DISABILITY INSURANCE, LIFE
INSURANCE, AND SUPPLEMENTAL LIFE AND DISABILITY AFTER 90 DAYS OF
SERVICE.
ALMY'S RETIREMENT SAVINGS PLAN
After one year of service, on specific plan entry dates, Almy employees
are eligible for our 401(k) plan. This plan allows employees to defer
part of their pre-tax salary for retirement. In addition, CM Almy
will contribute 100% matching funds up to 2% of salary. Almy also makes
a 5% of salary profit sharing contribution for all eligible employees.
TRAVEL ACCIDENT INSURANCE
Almy employees are covered by accidental death and dismemberment
insurance for all traveling they do on company business. The benefit
amount is based on salary level and the specific injury. Almy pays for
100% of this benefit. Almy employees are eligible for travel accident
insurance as of the first day of employment.
EMPLOYEE FITNESS BENEFIT
After three months of employment, Almy employees are eligible for a
reimbursement of approved fitness expenses. Reimbursement amounts vary.
EMPLOYEE REFERRAL PROGRAM
Almy employees are eligible for $500.00 if they refer a new employee who completes the 60 day introductory period.
EMPLOYEE ASSISTANCE PROGRAM
All Almy employees and their immediate families are eligible for
counseling and referral services through our Employee Assistance
Program. This benefit is paid 100% by CM Almy.
FULL AND PART TIME EMPLOYEES ARE ELIGIBLE FOR ALMY'S RETIREMENT SAVINGS
PLAN, TRAVEL ACCIDENT INSURANCE, EMPLOYEE FITNESS BENEFIT, EMPLOYEE
REFERRAL PROGRAM, AND EMPLOYEE ASSITANCE PROGRAM.
